| Before the Walk · Walkers 16 and under must be accompanied by an adult. · The “Petwalk” is a fund-raising event for the benefit of the animals and the animal welfare programs at the Humane Society. All walkers must have sponsors. Ask you friends, relatives and local merchants to sponsor you for the walk. · Have your sponsors fill out the sponsor sheet. Advise your sponsors that all pledges are due immediately so you may turn them in on the morning of the walk. · You are encouraged to sign up as many sponsors and seek the most pledges possible. Sponsors will pledge one amount for the entire walk. · Prizes will be awarded to walkers raising the most funds. · If additional sponsor sheets are needed they may be picked up at the shelter or downloaded from our website at www.hslc.pondpets.com · Be sure your dog is in shape for the walk, is wearing a suitable collar and is kept on a leash at all times. Dogs must be at least 6 mos old, vaccinated for parvo, distemper and rabies and have a valid dog license. No females in season will be allowed. |

Waiver: In consideration of my signing this agreement, I hereby for myself, my heirs and administrators assume any and all risk that might be associated with this walk.
